Well here are some basics before we actually go into the details. We begin with...

Game Design document (GDD):
The primary goal is to understand the challenges & the solutions involved in Game. The GDD is the first step in that process, after the Game idea & concept designs is formalized.

Yes it is a written document which describes in detail every aspect of the game, so the team will have a clear plan & set of instructions to follow.

The document needs to describe your game's look, the tone, the gameplay, the user interface, and go into every detail on what the game will be. It needs to be thorough and entertaining to read.
The GDD includes explanation of the game, Concept, Gameplay, Game Mechanics, Game Levels, Audio and assets in detail.

GDD Outline:

Title:
It is the starting point of the doc. The first page should start with the game’s title and the name of the company.

The Game:
The explanation of what the game is all about and what is its genre should be explained in brief.

Objective:
What the player has to achieve in the game has to be stated. Explanation of the player's goals and why would they want to accomplish it, is given in brief.

Story:
A brief description of story is given. Almost all current 3d games have a story line.

Gameplay:
It is the heart of the game. A game is revolves around its Gameplay. Gameplay is designing the set of goals, challenges and the options that the player has while playing the game. It explains what the player gets to do in the game, his aim and the result that he achieves by executing that aim. It is basically designing the rules of the game.
E.g.: the player has to kill all the enemies and complete the level.

Game mechanics:
A game mechanic is a construct of rules intended to produce an enjoyable game or game play. Game play and game mechanics are different. It is the description of the game play in detail. It explains a lot of technicalities. It gives clearer visual representation of the gameplay.

Game world:
Where the game does takes place, I mean in desert, mountains, forests or any hi-tech base. It is the description of the locations of the game including the sketches.

Game levels:
Each and every level is described in brief. Every level has a goal for the player and by accomplishing it player goes to another level.

Game Characters:
Character designs and description of all the characters, their role in the game and whom does the player play, is described in detail.

Controls:
It is the description of the User interface. How will the user/Player be able to navigate his way thru the game?

Game Audio:
The audio should go with gameplay. A game however is boring without any audio. In the game industry there are sound engineers to design the audio of the game. The description of every sound that you hear while playing the game is categorized and described here. Let it be the sound of your players walk or the sound of the bullet you fire, everything is thought upon and listed.
